ABSTRACT

Objetivo: refletir sobre os atos administrativos do Ministério da Educação do Brasil relacionados ao Estágio Curricular Supervisionado na área da saúde durante a pandemia do COVID-19 e as repercussões na enfermagem. Método: estudo com abordagem teórico - reflexivo, com base na literatura sobre Estágio Curricular Supervisionado no Brasil, tendo como referencial oito portarias governamentais na área da educação que se apresentam como medidas resolutivas para o enfrentamento aos desafios do contexto da pandemia. Resultados: foram explicitadas as intencionalidades subjacentes aos atos administrativos do Ministério da Educação, as lacunas, fragilidades e desafios na realidade do Estágio Curricular frente aos cenários das práticas de ensino no contexto da pandemia e as possibilidades de atividades remotas. Considerações finais e repercussões para a prática: espera-se que as mudanças, incertezas e desafios, destacados neste estudo, despertem cautela na tomada de decisão e condução de alternativas de adequações para o Estágio Curricular Supervisionado em enfermagem no contexto de pandemia e no percurso pós-pandemia


Objective: Reflect upon the administrative acts of the Brazilian Ministry of Education concerning Internship Programs in the health field during the COVID-19 pandemic and the consequences for the nursing field. Method: this study, with a theoreticalreflective approach, was based on the literature addressing Internship Programs in Brazil, considering eight ordinances in the field of education, intended to present decisive measures to deal with the challenges imposed by the pandemic. Results: The purposes underlying the administrative acts issued by the Ministry of Education, gaps, weaknesses, and challenges imposed to Internship Programs within the settings in which these programs are implemented are discussed considering the pandemic context and the possibility of implementing remote teaching modalities. Final considerations and repercussions for the practice: The changes, uncertainties, and challenges discussed in this study are expected to encourage prudence in decision-making when considering alternatives to adapt Nursing Internship Programs during and after the pandemic

ABSTRACT

La insuficiencia renal crónica es una enfermedad no trasmisible que se caracteriza por una disfunción renal que causa complicaciones y alteraciones metabólicas, las cuales disminuyen la capacidad funcional y la percepción de la calidad de vida relacionada con la salud de los pacientes. La atención de los pacientes con esta enfermedad demanda de cuidados integrales; entre los cuales los cuidados de enfermería ocupan un papel fundamental. El proceso de atención de enfermería es una herramienta necesaria para ofrecer el máximo nivel de calidad en los cuidados que recibe el paciente con enfermedad renal crónica en tratamiento de hemodiálisis. El objetivo de la presente investigación es exponer las particularidades del programa de atención de enfermería incluidas en el proceso de atención de enfermería de pacientes con insuficiencia renal crónica sometidos a tratamiento de hemodiálisis(AU)


Chronic kidney failure is a non-communicable disease in which there is kidney dysfunction that generates a significant number of complications and metabolic disorders, these decrease the functional capacity and the perception of quality of life related to the health of patients. The care of patients with this disease demands comprehensive care; within which nursing care plays a fundamental role. The nursing care process is a necessary tool to offer the highest level of quality in the care received by patients with chronic kidney disease undergoing hemodialysis. The objective of the present investigation is to contribute the elements from the pedagogical teaching point of view that guarantee an adequate knowledge of the actions to be carry out during the nursing care of patients with chronic kidney disease(AU)

ABSTRACT

Objetivo: avaliar a cultura de segurança organizacional de um hospital universitário português a partir da percepção dos enfermeiros. Método: estudo transversal, tipo survey, com 567 enfermeiros de um hospital universitário do Porto, Portugal, realizado de abril a dezembro de 2014. Utilizou-se o instrumento do Hospital Survey on Patient Safety Culture. Para análise, realizou-se a recodificação da Escala de Likert e aplicou-se a estatística descritiva. Resultados: as dimensões que apresentaram percentual elevado de respostas positivas foram: Trabalho em equipe dentro das unidades (78,69%) e Expectativa e ações de promoção de segurança do paciente dos supervisores (65,94%). Evidenciou-se que 248 (44,4%) profissionais não notificaram nenhum evento e apenas 13 (2,3%) notificaram mais de seis eventos no último ano. Conclusão: a avalição permitiu identificar uma dimensão forte e algumas fragilidades da organização. Os resultados são relevantes e servem de embasamento para os gestores e líderes de enfermagem, permitindo avançar na cultura de segurança.


Objective: to evaluate the organizational safety culture of a Portuguese university hospital on the basis of nurses' perceptions. Method: this cross-sectional, survey-type study of 567 nurses at a university hospital in Porto, Portugal, was conducted from April to December 2014, using the Survey on Patient Safety Culture instrument. For the analysis, the Likert Scale was recoded, and descriptive statistics were used. Results: the dimensions that returned high percentage positive responses were: Teamwork within units (78.69%) and Supervisor Expectations and Actions Promoting Patient Safety (65.94%). It was found that 248 (44.4%) of personnel did not report any event, and only 13 (2.3%) had reported more than six events in the prior year. Conclusion: the assessment identified one strong dimension in the organization, and some weaknesses. The results are significant and offer a foundation for nursing managers and leaders to improve safety culture.

Resultados: Las dimensiones que presentaron un alto porcentaje de respuestas positivas fueron: Trabajo en equipo dentro de las unidades (78,69%) y Expectativa y acciones de promoción de seguridad del paciente de los supervisores (65,94%). Se evidenció que 248 (44,4%) de los profesionales no notificaron ningún evento y sólo 13 (2,3%) notificaron más de seis eventos el último año. Conclusión: la evaluación permitió identificar una dimensión fuerte y algunas fragilidades de la organización. ABSTRACT

Objective@#A graded response system based on pediatric early warning score (PEWS) was constructed to provide a scientific basis for clinical intervention and management by clinical nurses.@*Methods@#Domestic and foreign literature and the results of previous research were reviewed to build a preliminary framework of graded response strategies. Two rounds of expert consultation were conducted by Delphi method and the results were statistically analyzed. Meanwhile, the results were integrated into the electronic medical record system to form the PEWS grading response system.@*Results@#The positive coefficient of experts in the first and second rounds of consultation both were 15/15, and the average of the expert authority coefficient was 0.87 and 0.89 respectively. The expert coordination degree Kendall′s W value was 0.292 and 0.301 respectively (P<0.05), indicating that the experts′ opinions tended to be consistent. Finally, four segmented response systems were formed, with PEWS scored on the scale of 0-1, 2, 3 and 4 or single 3 points.@*Conclusions@#The graded response system based on pediatric early warning score constructed by using Delphi expert consultation method can be used in clinic, which can promote the nurses′ participation in clinical intervention decision-making and improve the quality of nursing.

ABSTRACT

Objective To investigate the effects of precautionary rescue nursing intervention on the nursing quality score and efficacy of rescue for neonates with hypoxic ischemic encephalopathy. Methods One hundred children with neonatal hypoxic ischemic encephalopathy admitted to the First Hospital of Hebei Medical University from January 2017 to December 2018 were enrolled. Fifty children from January to December 2017 were assigned in the routine rescue nursing group; and 50 children from January to December 2018 were in the precautionary rescue nursing group. The routine rescue nursing group received routine rescue nursing; and the precautionary rescue nursing group was given pre-planned rescue nursing intervention. The rescue success rate, rescue time, Apgar score within rescue for 5 minutes, poor prognosis, etc. were recorded in both groups; the infant intelligence development scale was used to evaluate the situation of children's development of IQ in 6 months; a self-made questionnaire was used to investigate the rate of satisfaction in 100 children family guardians; the hospital's self-made Nursing Quality Assessment Form was applied to assess the quality of care of nursing staff. Results The rescue successful rate and the 5-minute Apgar score of the precautionary rescue nursing group were significantly higher than those of the routine rescue nursing group [rescue success rate: 98.0% (49/50) vs. 72.0% (36/50), Apgar score: 8.43±1.28 vs. 5.74±1.39], and the rescue time and poor prognosis rate were significantly lower than those in the routine rescue nursing group [rescue time (minutes):3.18±1.20 vs. 6.78±2.23, poor prognosis rate: 2.0% (1/50) vs. 14.0% (7/50), both P < 0.05]. The 6-month intelligence index and psychomotor index in the precautionary rescue nursing group were significantly higher than those in the routine rescue nursing group (intelligence index scores: 96.43±5.85 vs. 85.40±8.90, psychomotor index scores:97.38±4.03 vs. 78.43±10.22, both P < 0.05). The satisfaction rate in precautionary rescue nursing group was obviously higher than that in the routine rescue nursing group [92.0% (46/50) vs. 76.0% (38/50), P < 0.05]; the rescue technology, rescue efficiency, rescue effect, humanistic care score in precautionary rescue nursing group were significantly higher than those in the routine rescue nursing group (8.12±0.89 vs. 2.74±1.02, 8.23±1.24 vs. 3.01±0.83, 7.80±1.33 vs. 3.44±1.25, 8.03±1.32 vs. 3.29±1.38, respectively all P < 0.05). Conclusion The precautionary rescue nursing intervention used for neonates with hypoxic ischemic encephalopathy can significantly improve the rescue effect, degree of satisfaction to nursing and the quality of nursing care, so that it is worthy to be applied clinically.

Objective@#To explore the status of care quality evaluation in the built-in type institution that integrating pension and health care service and analyze the focus on nursing service and management,it aims to provide theoretical basis for nurse managers to implement scientific nursing management.@*Methods@#Descriptive phenomenological methodology was adopted in this study,a semi-structured interview was conducted with 8 nurse managers from 7 built-in type institutions that integrating pension and health care service.Data were analyzed based on Colaizzi′ s phenomenological research method.@*Results@#Two themes were extracted, including the status of care quality evaluation in the built-in type institution that integrating pension and health care service; key indicators of nursing quality in the built-in type institution that integrating pension and health care service.@*Conclusions@#Through in-depth interviews with 8 nurse managers, we suggest that state should attach importance to the quality inspection of nursing care in the built-in type institution that integrating pension and health care service, and explore the evaluation criteria for nursing quality,in order to ensure the safety of elderly care, and it also can provide a basis for the comparison between inter-agency nursing quality and promote healthy competition among institutions.

ABSTRACT

Objective To investigate the effect of Montelukast Combined with Symbicort in severe asthma treatment and intervention. Methods The treatment according to different interventions will be January 2015 to January 2017 in our hospital 80 cases of severe asthma groups:control group with routine nursing intervention for a letter form treatment, the observation group with high quality nursing the intervention of montelukast Symbicort treatment; detailed records of patients in the two groups of experimental data, and the data for comparative analysis, to explore the combination of montelukast and Symbicort in severe asthma treatment and intervention effect. Results The high quality nursing intervention meng montelukast Symbicort (observation group) for the treatment of severe clinical the effect of nursing intervention in patients with asthma is better than conventional Symbicort (control group) clinical efficacy, clinical symptoms improved better than the control group, nursing satisfaction was higher than the control group, the difference was statistically significant (P< 0.05). Conclusion The high quality nursing intervention of patients with severe asthma montelukast Symbicort treatment effect significantly, can effectively improve the clinical symptoms of patients, nursing satisfaction is high, it is widely used in clinical treatment of patients with severe asthma intervention.

ABSTRACT

Objective To explore the effect of Plan. Do-Check-Act on homogenization of nursing processes in ICU. Methods A total of 36 nurses were selected to implement Plan. Do-Check-Act in ICU according to evidence-based medicine. Their nursing was inspected by a senior nurse. The differences of the following items before and after Plan. Do-Check-Act were compared: nursing quality comprehensive quality control index and specialized quality control index. Results After the implementation of Plan. Do-Check-Act, comprehensive quality control index of clinical nursing quality score by (89.28 ± 2.36) points up to (97.45±1.38) points, and there was significant difference(t=-7.310, P<0.01). The specialized quality control indicators including the incidence of ventilator associated pneumonia, the incidence of catheter-related bloodstream infection, the incidence of catheter-associated urinary tract infections, the incidence of accidental extubation, the incidence of pressure sores, the incidence of glycemic out of control, the incidence of sedation out of control and retention time were 26.2%(53/202), 17.8%(36/202), 18.8%(38/202), 11.9%(24/202), 2.5%(5/202), 18.3%(37/202), 15.3%(31/202), (168.0 ± 3.3) h before the implementation and 7.1%(14/196), 3.1%(6/196), 4.1%(8/196), 0.5%(1/196), 0, 3.6%(7/196), 3.1%(6/196), (96.0±4.2) h after the implementation. There were significant differences (χ2=4.913-25.907, t=66.195, all P<0.01). Conclusions The Plan. Do-Check-Act is an effective way to improve nursing quality of ICU to make it homogeneous, reduce adverse nursing and iatrogenic complications.

Methods The control group was given routine nursing, the observation group was given quality nursing care on the routine nursing. Results Effects of nursing intervention in the observation group was better than the control group, and the improvement of patients psychological status, and pulmonary function was better than the control group with statistical significance (P<0.05). Conclusion Doxofylline plus Budesonide plus routine nursing and quality nursing care intervention in patients with bronchial asthma could quickly improve the psychological status of patients, relieve / eliminate pulmonary dysfunction with significant efficacy.

Objective To understand the current situation of neurology nurses′self-efficacy and quality of nursing services in Lanzhou area, and explore the relationship between self-efficacy and the quality of nursing services. Methods Using the method of questionnaire, one hundred and the neurology nurses and hospitalized patients in 5 Grade 3 Level hospitals of Lanzhou city were investigated by using self-efficacy scales and Servqual scales. Results Neurology nurses′ self-efficacy in Lanzhou area was lower than the general population ( U=-9.875,P=0.042);the scores of quality of care from high to low were at reliability, tangibles, assurance, responsiveness, cost acceptability and empathy, which were-(0.12 ± 0.05) points,-(0.23 ± 0.03) points,-(0.24 ± 0.06) points,-(0.25 ± 0.02) points,-(0.26 ± 0.05) points,-(0.66 ± 0.03)points.Satisfaction arranged from high to low were assurance (89.98%), reliability (89.69%), responsiveness (89.12%), tangibles (87.66%), cost acceptability (87.08%), empathy (86.67%).There were positive correlations between satisfaction and expectations and perceived service quality, and the correlation between satisfaction and perceived qualities of service was higher. Moreover, there might exist positive correlation between self-efficacy score and quality of nursing services (reliability, assurance, responsiveness, empathy) (P values were 0.002, 0.001,0.012 and 0.005). Conclusions Neurology nurses in Lanzhou area have lower self-efficacy, and the perceived qualities of care from patients never exceed their expectations. As results, hospital managers should take measures to improve general self-efficacy of nurses, and thus enhance the quality of care.

ABSTRACT

Objective To describe the prevalence and change of rationing of nursing care (RNC) in medical and surgical units of level 2 and level 3 general hospitals across Guangdong province. Methods RNC were measured by asking nurses about the completion status of 12 necessary nursing care activities, and the prevalence of RNC in 2014 was compared with the study results from the China Hospital Nursing Workforce Research(CNWR) in 2009 (exclusively using the data of Guangdong province) to describe the change of RNC. Results Six activities were rationed averagely in 2014, and the most frequent activities rationed wereComfort/talk with patients accounted for 69.8% (975/1 396) andEducating patients and familiesaccounted for 67.3%(940/1 396). According to the CNWR in 2009, the average number of nursing care activities rationed was 3 which was statistically lower than the number in 2014 (P < 0.01). Conclusions RNC is prevalent in level 2 and level 3 general hospitals across Guangdong province, especially psychosocial care to the patients; the prevalence of RNC has not decreased since 2009.

ABSTRACT

PURPOSE: The objective of this study was to investigate the influence of hospital nurses' self-leadership, communication skill, and self-esteem on the quality of nursing service. METHODS: The participants, 230 nurses working at a general hospital in Seoul, completed a cross-sectional descriptive questionnaire survey between January 17 and 28, 2014. The collected data were analyzed using the SPSS 19.0 program for descriptive statistics, t-test, one way & two way ANOVA, correlation, and multiple regression analysis. RESULTS: Nurses' self-esteem and communication skill had significant main effects on self-leadership and the quality of nursing service, but the interaction effect of the two independent variables was not significant. Variables that significantly influenced the quality of nursing service were self-leadership, communication skill, self-esteem, and career longevity. The explanatory power of these variables for the quality of nursing service was 54.4%. CONCLUSION: Findings indicate a need for education programs for nurses which are designed to promote communication skill and enhance self-esteem and self-leadership skills which will in turn enhance the quality of nursing service.

ABSTRACT

Background: Nursing is experiencing an unprecedented shortage of skilled professionals as supply dwindles and demand escalates. For this reason, organizations are diligently trying to understand both what attracts nurses to a certain employer and what retains nurses in an organization. The purposes of this quasi-experimental design were to study the perceptive level of professional nurses and compare the pretest and posttest the use of nursing workplace empowering model on quality of nursing work life. Methods: The Quality of Nursing Work Life Scale was used with total reliability coefficient at = 0.843. Regarding to the study's model, the professional nurses of experimental group have been used for daily work. The Mann-Whitney tested for comparing the experimental group and control group. Moreover, the Wilcoxon Signed-Rank Test was used to compare the experimental group and control group and before and after implementing the nursing workplace empowering model. Results: The major findings indicated that the professional nurses who used NWEM were significantly higher scores than who did not use the NWEM at .05 level. In addition, the experimental group after implemented the NWEM was higher scores than before implementing NWEM at 0.05 level. Conclusions: The nurses' perceptions of structural and psychological empowerment are significantly increased flexibility, relaxation, free expression, and support. The structural and psychological empowerment had a direct effect on the all variables of the model.

Objective To know the effects of using hierarchical management for psychiatric nursing in psychiatric ward,and then to offer some rationales and theoretical support for hospital management.Methods The fine hierarchical management was implemented from Janurary 2012 in Xi'an Mental Health Center and the metal hygiene clinics of Xi'an Jiaotong University.Before and after the hierarchical management carried out,the nursing care quality was assessed among the family members of hospitalized patients by using satisfaction survey questionnaires,flight rate of patients was evaluated,and at the same time,nursing care quality and the nurse comprehensive ability was inspected.And then,the EPIDATE 3.0 and SPSS16.0 software was used to analyze and compare the satisfaction scores.The nurse comprehensive ability,and nursing care quality scores before and after implementation.Results The nurse comprehensive ability were improved.The satisfaction scores of family members of hospitalized patients was higher than that before implementation after the fine management implementation,which had statistical difference,P< 0.05.The satisfaction rate of patients' family member was 96.77%(2 543/2 628) after the fine management implementation,which was significant higher than that of before implementation,91.19%(2 382/2 612),x2=107.66,P<0.05.Flight rate of patients was reduced after the hierarchical management,which was significantly lower than that of in before,P<0.05.Nursing care quality scores were higher than those of in before,which has a statistical difference respectively,P<0.05.Conclusions The hierarchical management could improve the satisfaction of family numbers,reduce the flight rate of patients,enhance nursing care quality and improve the nurse comprehensive ability.It would reduce the incidence of medical malpractice.The fine hierarchical management is worth to be applied and generalized in nursing care of psychiatric nursing in psychiatric ward.

ABSTRACT

Objective To translate the English version of the Oncology Patients' Perceptions of the Quality of Nursing Care Scale (OPPQNCS) into Chinese and to test the reliability and validity of the Chinese version of OPPQNCS in oncology patients.Methods The English version of OPPQNCS was translated by Brislin translation mode.Totally 320 oncology patients were recruited and were investigated by the Chinese version of OPPQNCS.Results The internal consistency coefficient of the Chinese version of OPPQNCS was more than 0.70,ranged from 0.804 to 0.964;the test-retest was 0.826.The content validity index(CVI) was 0.845-1.000,the average CVI was 0.925.Factor analysis got four factors,which explained 78.548% of the total variance.And every objective factor loading were greater than 0.4.Conclusions The Chinese version of OPPQNCS has been proved to be reliable and valid.It can be used as a valid tool for the oncology patients to measure their perceptions of the quality of nursing care they got.

ABSTRACT

Objective To introduce the method and experience of the setting of psychological care nurses,to discuss its effect on high-quality nursing and on amelioration of the patients' negative moods.Methods Psychological care nurses were set up in all the clinical departments,after systemic training of psychological knowledge and became qualified psychological consultant via national certification examination.We took psychological care nurses as the leading role,assisted with other nursing modes.By the means of mental status scale in non-psychiatric settings (MSSNS) and the satisfaction questionnaire of psychological care to evaluate the patients' mental status and satisfaction degrees before and after the intervention in 297 cases.Results Patients'scores of anxiety scale,depression,anger,loneliness factor and the total score of the MSSNS after the psychological nursing intervention were significant differences than those before the psychological nursing intervention,[(39.66±4.70)vs (45.59±6.45),(23.31±2.28)vs (26.59±3.35),(13.91 ±3.01) vs (15.12±2.58),(17.58±3.74)vs (20.29 ±4.67),(92.09±6.85)vs (106.66±8.58),t values was 14.376,15.213,23.992,13.974,13.641 respectively,P<0.05].The satisfaction rate of patients for psychological nursing intervention after the intervention was 96.97%(288/297),which was significant higher than that before the intervention,69.70%(207/297),x2=79.53,P<0.05.Conclusions Psychological care demands professional qualities.Only through systemic training,can grasp the nurses master professional psychological knowledge and skill,so as to apply scientific psychological care to the patients and help them to solve their psychological perplexities and maintain physical and psychological well-beings.

ABSTRACT

Objective To explore osculating value method combined with rank sum ratio method in evaluation of nursing quality in the hospital nursing management application, to provide a scientific basis for decision making. Methods By reference, from the working efficiency, the quality of nursing, 3 dimensions of nursing skill training, the choice of 11 commonly used in the evaluation of nursing quality indicators, the raw data of each index was normalized, the school of nursing quality with 2004-2013 years of osculating value method analysis was assessed, the development of nursing quality combined with rank and file classified score method. Results The nursing quality of the overall upward trend, according to the grading each year osculating value and ranking in 2011, nursing quality best, osculating value was 0.000, the worst quality of nursing in 2004, osculating value was 3.248;the annual nursing quality grading was reasonable, 2011, 2012 and 2013 were in good quality grade, 2004, 2005 and 2006 were of poor quality grades, the rest of the year at a medium level, with statistically significant difference of the file, P<0.05. Conclusion Osculating value method combined with rank sum ratio method in comprehensive evaluation of nursing quality in the hospital has the objectivity and practicability.

Objective:To explore the clinical effect of high quality nursing service in department of neurosur-gery.Methods:The patient and family's satisfaction degree , number of nursing errors , medical disputes , family ac-company rate and so forth were compared before and after the implementation of high quality nursing service in de-partment of neurosurgery .ResultsAfter the implementation of high quality nursing , the satisfaction degree of pa-tients and their families was improved and the nursing errors , medical disputes and family members accompany rate were reduced .Conclusions:High quality nursing service mode could improve the nursing quality and patient satis-faction in department of neurosurgery .
